Our interdisciplinary team focuses on dignity, comfort, and quality of life while supporting patients and their families through every stage of the hospice journey. Position Summary We are seeking a highly skilled and responsive Admission / Resource Nurse (RN) to join our team full-time. This role is critical in ensuring timely admissions, seamless care transitions, and clinical support across the organization. The ideal candidate is clinically strong, organized, and thrives in a fast-paced, collaborative hospice environment. Key Responsibilities
  • Admissions & Intake Coordination
  • Respond promptly to new hospice referrals (facility, hospital, home, and community-based)
  • Perform comprehensive admission assessments, including physical, psychosocial, and environmental evaluations
  • Explain hospice philosophy, services, and consent process clearly to patients and families
  • Complete all required admission documentation accurately and in a timely manner
  • Collaborate with intake coordinators, physicians, and referral sources to ensure smooth transitions of care Clinical Resource Support
  • Serve as a clinical resource for field staff, providing guidance on patient care and symptom management
  • Assist with urgent visits, evaluations, and patient support as needed
  • Support case managers with complex clinical situations and care planning
  • Ensure compliance with hospice regulations and best practices Care Coordination
  • Initiate and update the Plan of Care (POC) in collaboration with the interdisciplinary team
  • Coordinate DME, medications, and supplies at the time of admission
  • Communicate effectively with physicians, facilities, and internal team members
  • Ensure continuity of care from admission through ongoing services Quality & Compliance
  • Maintain accurate and timely documentation in the EMR system
  • Adhere to Medicare/Medicaid hospice Conditions of Participation and organizational policies
  • Participate in quality improvement initiatives and team meetings
  • Qualifications & Requirements
  • Active and unrestricted Registered Nurse (RN) license in the State of Illinois
  • Minimum 1 year of nursing experience (hospice, home health, or acute care preferred)
  • Prior hospice admission or intake experience strongly preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage time effectively
  • Proficiency with electronic medical records (EMR systems)
  • Valid driver's license and reliable transportation Preferred Skills
  • Knowledge of hospice regulations and end-of-life care practices
  • Experience working with interdisciplinary teams
