  • Friday; 7:30am or 8am start time and 4:30pm or 5pm end time (8.
5 hour days) PRN schedule: at least 2 shifts every 6 weeks Weekend rotation No holidays and no call Additional Preferred Requirements At least 2 years of nursing experience (inpatient or outpatient) Prior Case Management experience Overview Memorial Hospital Belleville is an acute care hospital offering medical and surgical services plus critical and emergency care including Children's at Memorial for pediatric emergency care. It provides patients a full complement of diagnostic and treatment services as well as heart and vascular care. Memorial, offering medical and surgical services plus critical care, is an accredited Chest Pain Center with PCI by the Society of Cardiovascular Patient Care and is designated as an Acute Stroke Ready Hospital by the Illinois Department of Public Health. In addition, Memorial Belleville recently was accredited by the American College of Radiology as a designated Lung Cancer Screening Center. Since 2008, it has been designated as a Magnet
  • recognized organization for nursing excellence by the American Nurses Credentialing Center.
Memorial Hospital Shiloh , a 94-bed, all-private suite hospital was recognized with Magnet status in 2018 and provides emergency care, labor & delivery, nursery, medical and surgical services plus critical care. Care Management includes Utilization Management (Utilization Review), Discharge Planning, Social Services, Clinical Outcomes (Care Managers), Discharge Transition Coaches, Nurse Practitioners, and Clinical Documentation Improvement. Preferred Qualifications Role Purpose Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source. Responsibilities Assesses, coordinates, and evaluates each patient's plan of care. Establishes, continuously evaluates, and documents an individualized discharge plan. Ensures clinical documentation and cost-effective use of hospital resources. Minimum Requirements Education Nursing Diploma/Associate's
