Responsibilities Include:

Ability to think critically and make decisions when caring for patients in their home environment. Follow individualized care plans and document patient progress. Provide direct care to patients, administer medications, monitor vital signs, perform physical assessments and treatments. Communicate effectively with patients, families, and the healthcare team. Complete electronic documentation and paper documentation that maintains compliance with all state and federal guidelines. Perform physical tasks, including lifting and mobility assistance Participate in continuing education as required. Provide compassionate care in the patient's home, fostering a supportive, respectful environment that prioritizes their well-being.

Qualifications Include:

Active Indiana or Compact RN/LPN license in good standing Graduate from an accredited Nursing program CPR certification Familiarity with HIPAA standards. Minimum of 1 year nursing experience Strong clinical assessment skills Confident working independently and managing patient care in a home setting Candidate must be able to lift 50 pounds and pass a fit to work screen Apply Today!
