Position Summary The MDS Coordinator is responsible for completing resident assessments, developing care plans, and ensuring accurate reimbursement in compliance with Medicare and Medicaid guidelines. Key Responsibilities Complete and manage resident MDS assessments, including CAAs and triggers Coordinate interdisciplinary care plan meetings and ensure timely updates Develop and revise individualized care plans based on resident needs Support Medicare/Medicaid reimbursement processes and ensure accurate RUG coding Review clinical documentation to ensure compliance and proper billing Lead interdisciplinary care plan team meetings Conduct audits to ensure accuracy, timeliness, and compliance of MDS documentation Support survey readiness and respond to regulatory requests Train staff on RAI process and documentation requirements Maintain compliance with all federal, state, and facility policies Qualifications Registered Nurse (RN) or Licensed Practical Nurse (LPN) per state/facility requirements Completion of an accredited nursing program MDS, care planning, and assessment experience preferred (long-term care experience strongly preferred) Strong attention to detail and knowledge of Medicare/Medicaid guidelines Excellent communication, organization, and documentation skills Ability to work collaboratively with interdisciplinary teams Additional Requirements Ability to lift up to 75 lbs and perform physical tasks as needed Must meet all state and federal regulatory requirements Strong interpersonal skills with energy, professionalism, and teamwork focus Equal Opportunity Employer (EOE) MDS Coordinator 4.3 4.3 out of 5 stars 501 South Murphy Avenue, Brazil, IN 47834 Full-time HutsonWood at Brazil 9 reviews Full-time Why Join Us We believe strong benefits support a strong life outside of work.
