We are seeking a compassionate Nurse Liaison who enjoys working with families, assessing client needs, and coordinating care that improves quality of life. This role combines clinical assessment, relationship building, and care oversight. What You'll Do Conduct discovery calls with prospective clients to understand care needs Perform in-home client assessments and develop individualized Plans of Care for Clients Educate families about care options and expectations Monitor client condition and update care plans as needs change Provide guidance to our Care Support Specialist supporting the client and caregivers Collaborate with hospice providers, physicians, and other healthcare partners Serve as a trusted resource for families navigating care decisions Conduct periodic reassessments to ensure care quality and satisfaction Qualifications RN licensed in Indiana Experience in home care, hospice, senior care, or case management preferred Strong communication and relationship-building skills Ability to assess client needs and develop care plans Reliable transportation for in-home visits Why Join Senior1Care?
