Job Overview Lane House is seeking a dedicated and detail-oriented MDS Coordinator - RN to oversee the completion and accuracy of Minimum Data Set (MDS) assessments for our skilled nursing residents. The ideal candidate will possess extensive knowledge of clinical documentation, coding, and regulatory standards to ensure compliance with Medicare, Medicaid, and other payer requirements. This role offers an opportunity to lead clinical documentation improvement efforts while supporting quality patient care through precise data management. Duties Coordinate and facilitate comprehensive MDS assessments in accordance with federal and state regulations, including CMS guidelines. Collaborate with interdisciplinary teams to gather accurate clinical information necessary for MDS completion. Ensure timely submission of MDS assessments while maintaining compliance with all regulatory deadlines. Review medical records, documentation, and coding (including
ICD-10, CPT, ICD-9
) to verify accuracy and completeness. Conduct ongoing education for staff on MDS updates, coding standards, and documentation best practices. Support utilization review and management initiatives by analyzing data to optimize patient care plans and reimbursement strategies. Maintain proficiency in EMR (Electronic Medical Record) systems such as Epic, Cerner, or Athenahealth to streamline documentation processes. Assist in discharge planning and coordination of post-acute services like home care or hospice when applicable. Qualifications Registered Nurse (RN) licensure with current state license required. Proven experience with MDS coordination within a skilled nursing or long-term care setting. Strong understanding of Medicare regulations, NCQA standards, and healthcare compliance requirements. Knowledge of
ICD-10, ICD
coding systems, CPT coding, and medical terminology essential for accurate assessments. Experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth preferred. Critical care experience or ICU background highly desirable to interpret complex clinical data effectively. Familiarity with managed care processes, utilization review, and clinical documentation improvement initiatives. Excellent organizational skills coupled with attention to detail and the ability to manage multiple priorities efficiently.
