• Responsibilities

What You'll Do:

+ Implements the medical plan of treatment and follows physicians' orders. + Monitors individual at all times, following all CHC and agency safety protocols. + Documents activities and services accurately and timely according to CHC and agency regulations. + Monitors general health conditions of the individual and notifies supervisor of any medical issues or if treatment is needed. + This description reflects assignment of essential functions, management may assign or reassign duties and responsibilities to this job at any time that are not listed above.

What You'll Bring:

+ Ability to work in a professional, ethical, and respectful manner. + Ability to provide personal care for non-ambulatory clients, use a Hoyer lift, maneuver wheelchairs, and transport clients to community events and social activities. + Strong communication skills, oral and written; strong interpersonal skills. + Ability to anticipate and resolve problems demonstrating good judgment.

Education and Experience:

+ Current RN license. + Current CPR/First Aide certification. + One (1) year of experience providing skilled care to the medically fragile.

Travel Requirements:
