Home Health Registered Nurse (RN or BSN)

We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide high quality, patient - centered care in the comfort of patients' homes. This role focuses on promoting independence, managing chronic conditions, and supporting recovery after illness or surgery.

Responsibilities:

Perform comprehensive in-home patient assessments Develop and implement individualized care plans Administer medications, post surgical care and treatment as prescribed Monitor patient conditions and report changes to CNM Coordinate care with physicians, therapists, and other team members

Qualifications:

Active RN license in the State of Indiana Minimum of 1-2 years' experience (Home Health preferred) Strong Clinical Assessment skills Must be a critical thinker Self -Starter Ability to work independently and manage time effectively Excellent Communications skills

Requirements:

Valid State of Indiana Driver's License and reliable transportation Current CPR certification Ability to travel within assigned service area Ability to lift/ move patients as needed

Work Environment:

Patient home within Indiana area Flexible scheduling

Pay:

$31.50 - $41.50 per hour
