Job Summary We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality home health and home care services to patients in their private residences. The RN will be responsible for delivering skilled nursing care, coordinating patient treatment plans, and educating patients and families to promote health, recovery, and independence in the home setting. Responsibilities Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans Perform comprehensive patient assessments and document findings accurately Administer medications, injections, IV therapy, and wound care as required Monitor patient condition and report changes to the physician and care team Develop, implement, and update individualized care plans Educate patients and caregivers on disease management, medications, and home care techniques Coordinate care with physicians, therapists, and other healthcare professionals Maintain accurate and timely clinical documentation in compliance with agency policies and regulations Follow infection control, safety, and confidentiality standards at all times Participate in case conferences and quality improvement activities as needed Qualifications Current Registered Nurse (RN) license in the state of practice Graduate of an accredited nursing program Minimum [1 year] of nursing experience preferred (home health experience a plus) Strong clinical assessment and critical thinking skills Excellent communication and interpersonal skills Ability to work independently and manage time effectively Valid driver's license, reliable transportation, and auto insurance Current CPR certification (BLS) Preferred Skills Experience in home health, hospice, or community-based nursing Familiarity with electronic health records (EHR) systems Knowledge of Medicare/Medicaid home health regulations Compassionate, patient-centered approach to care Must Have Wound care experience Tracheostomy (trach) care experience
