Job Description

Overview We are seeking a dedicated and compassionate Registered Nurse for our PACE Clinic. The Home Care/Clinic Coordinator RN ensures quality and safe delivery of nursing services in the PACE Clinic and in participants' homes. He or she assesses, plans, implements, and coordinates nursing care across settings. He or she coordinates care with primary care providers to ensure an efficient workflow. The Home Care/Clinic Coordinator primarily provides nursing visits in the PACE Center but can routinely visit participants in their homes, hospitals, nursing homes, and another alternate setting as deemed necessary for care coordination consistent with the PACE model of care. Duties Responsible for assessing, coordinating, monitoring, and providing nursing care services for participants in the clinic and the participants home. Responsible for initial and ongoing assessments and creation of nursing care plans. Coordinates and collaborates with the PACE Center Medical Director and members of the IDT regarding Infection Control and Surveillance activities at the PACE Center. Aids in education to participants regarding all PACE services. Addresses pharmacy issues as they occur. Administers medications to participants at the PACE Center and monitors participants' home medication use as necessary. Schedules outside appointments for testing or with specialists as needed. Manages the PACE Center laboratory, performing lab tests and conducting quality control testing as required. Provides cross coverage for the Home Care/Enrollment Coordinator. Participates in on-call rotation for after-hours participant needs. Administers medication and treatment as prescribed by the PCP. Provides prescribed or emergency care to participants requiring professional nursing services, including venipuncture, dressing changes, straight catheterization, suctioning, immunizations, etc. Performs diagnostic tests as authorized by the Primary Care provider (PCP). Obtain laboratory specimens when indicated. Provides treatment under medical direction. Assesses, evaluates, and reports to the PCP about the participant's physical and emotional condition, reaction to medications and/or treatments, and/or critical laboratory results requiring PCP intervention. Teaches the patient and family/caregivers self-techniques whenever appropriate and provides instruction regarding medication, diet, safety, and treatment modalities in accordance with the plan of care. Evaluates and promotes a safe environment for the participant and complies with National Patient Safety Goals. Responsible for completing all Clinic logs as per policy. Tracks all stock medications and medical supplies in the Clinic and ensures proper inventory and expiration date. Includes inventory of Emergency Crash cart. Places orders for resupply as appropriate. Responsible for the overall operation of the Clinic, including medication management, equipment maintenance, Hazard disposal, etc. Will be required to travel to participant's homes as needed. Qualifications Current unrestricted RN license in the state of practice, BSN preferred. Experience in home health care or similar settings preferred. Current BLS and or ACLS Proficient in using electronic health record (EHR) systems such as Epic or Cerner. Strong clinical skills in areas such as acute care, geriatrics, pediatrics, or critical care. Minimum of one (1) year general clinical experience in a certified home health agency, acute care, medical surgical, and/or critical care environment, or Nursing Home experience required. Clinical experience must be within one (1) year from date of hire or transfer, required. Minimum of one (1) year of experience working with the elderly. Customer Service experience is required.
