Job Description

Nurse Practitioner / Advanced Practice Provider•
Mishawaka, IN Compensation:
$120,000•$140,000+ base salary (range flexible based on experience; higher end available for language-critical areas) Performance bonus up to $8,000 Sign-on bonus flexible
Schedule:
Monday•Friday, 8 am•5 pm This position sits within a Medicare-focused value-based care model built around outcomes not volume. You'll join a fully supported care team, see a sustainable panel, and be rewarded for keeping patients healthy and out of the hospital. Bonuses are structured to reward low hospital admissions, preventive care, and strong patient experience.
Practice Details Patient volume:
13•16 per day Visit length: 20 minutes standard / 40 minutes for new patients and full diagnostics Visit frequency: approximately every 3 months (complex cases seen more frequently) Care team: Scribe, Medical Assistant, Social Worker On-site resources: In-house lab with phlebotomist, RubiconMD specialist access
EMR:
Canopy and Greenway Providers per clinic: 3•5 (mix of NPs and PCPs) Patient transportation: Van pickup available within 5 miles of clinic Community room with patient programming: exercise classes, yoga, spa days, and social events Benefits 6 weeks PTO, inclusive of major holidays and CME time $5,000 CME stipend Full health, dental, vision, and life insurance 401(k) with up to 4% company match, immediately vested Malpractice insurance provided Tuition reimbursement Qualifications MSN, DNP, or NP Certificate Program graduate National certification in one of the following: Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP Active, non-probationary state NP license Active DEA license HCC documentation, ICD-10 coding, and HRA experience preferred If you're interested in learning more or exploring this opportunity, we'd love to connect.
