- Full-time Overview We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team.
The ideal candidate will possess a strong background in acute and post-acute care settings, with expertise in patient assessment, discharge planning, and utilization management. This role offers an opportunity to coordinate comprehensive patient care, improve health outcomes, and ensure efficient healthcare delivery across various clinical environments including inpatient, outpatient, and clinic settings. The Registered Nurse Case Manager will play a vital role in bridging clinical care with case management strategies to support patient recovery and continuity of care. Duties Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations. Develop individualized care plans based on comprehensive diagnostic evaluations and patient needs. Coordinate discharge planning and post-acute care arrangements to facilitate smooth transitions from hospital to home or other facilities. Manage utilization review and ensure appropriate level of care in compliance with healthcare policies such as HIPAA. Collaborate with multidisciplinary teams including physicians, therapists, social workers, and other healthcare professionals. Monitor patient progress through ongoing evaluation of medical records and clinical documentation. Provide patient education on health management, medication adherence, and follow-up care instructions. Assist in managing complex cases involving critical care, trauma (Level I & II Trauma Centers), ICU, PICU, ER experience, behavioral health, geriatrics, infectious disease care, or surgical units such as OR. Support intake processes for outpatient clinics or inpatient admissions while ensuring compliance with hospital standards. Participate in quality improvement initiatives related to patient service and case management efficiency. Qualifications Valid RN license with current registration; experience in hospital medicine or acute care settings preferred. Critical care experience including ICU, ER, trauma centers (Level I & II), or surgical units such as OR highly desirable. Knowledge of medical terminology, physiology, diagnostic evaluation procedures, and HIPAA regulations. Proven experience in discharge planning, utilization management, case management, and post-acute care coordination. Strong skills in patient assessment and medical records documentation. Experience working within managed care environments or outpatient clinics is advantageous. Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams. Ability to handle complex cases involving pediatrics (PICU), geriatrics, behavioral health, infectious diseases, or outpatient services. Demonstrated ability to adapt quickly to diverse clinical situations including inpatient and outpatient settings.
