Clinical Supervisor Job Summary Altra Home Health is seeking a compassionate and skilled Nurse to join our team. In this rewarding role, you'll provide personalized care to pediatric patients in their homes — many of whom have complex medical conditions , including gastrostomy tubes (G-tubes), cerebral palsy, chronic diseases , and occasionally require tracheostomy or ventilator support . You'll play a vital role in helping them live safely and comfortably in their home, where they thrive the most. Responsibilities Perform thorough head-to-toe assessments and follow individualized plans of care Provide care which may include G-tube feedings, medication administration, wound care, trach care and ventilator support. Care for patients with professionalism and empathy.
