Through regular nurse home visits starting during pregnancy and continuing until the child's second birthday, we partner with families to provide education, guidance, and support that fosters lifelong health and stability. As a Nurse Home Visitor, you will deliver compassionate, relationship-centered care to first-time parents in their homes and communities. This unique role combines clinical nursing practice with coaching, education, and care coordination. You'll play a vital role in empowering families while working within the NFP model and collaborating with a multidisciplinary team dedicated to long-term impact.

Compensation:

$65,000

Responsibilities:

Apply the nursing process (assessment, diagnosis, planning, intervention, evaluation) to support maternal and child health outcomes Assess physical, emotional, social, and environmental needs and develop individualized care plans Establish trusting, therapeutic relationships using motivational interviewing and strengths-based, client-centered techniques Maintain thorough, timely documentation in the NFP data system and use data to inform practice Provide health education, parenting guidance, and referrals to community resources

Qualifications:
