Job Description

Job Overview We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, conducting comprehensive assessments, and developing individualized care plans to ensure optimal health outcomes. This role requires a strong background in various clinical settings, including critical care, trauma centers, emergency medicine, and post-acute care. The RN Case Manager will serve as a vital link between patients, families, and healthcare providers, ensuring seamless transitions across inpatient, outpatient, and community-based services. Candidates must demonstrate excellent patient service skills, thorough knowledge of medical terminology and physiology, and adherence to HIPAA regulations. Responsibilities Conduct thorough patient assessments to determine needs and develop personalized care plans. Coordinate multidisciplinary care involving physicians, therapists, social workers, and other healthcare professionals. Manage discharge planning to facilitate smooth transitions from hospital to home or post-acute care facilities. Oversee utilization management and diagnostic evaluation to optimize resource use while maintaining quality standards. Monitor patient progress through vital signs, medical records review, and ongoing evaluation of treatment effectiveness. Assist with intake procedures for new patients in both inpatient and outpatient settings. Ensure compliance with HIPAA regulations and maintain accurate documentation in medical records. Provide education to patients and families regarding treatment plans, medication management, and post-discharge instructions. Collaborate with hospital teams in high-acuity environments such as ICU, PICU, ER, or trauma centers to deliver specialized patient care. Participate in quality improvement initiatives related to patient safety and clinical outcomes. Experience Candidates should possess extensive nursing experience across diverse clinical environments including: Critical care units such as ICU or PICU with ICU experience preferred. Trauma centers (Level I or Level II trauma center experience). Emergency room or emergency medicine background. Medical-surgical units with strong patient assessment skills. Post-acute care settings including rehabilitation or skilled nursing facilities. Outpatient clinics or managed care environments with familiarity in outpatient services and primary care coordination. Additional qualifications include knowledge of hospital medicine, infectious disease care, behavioral health integration, geriatrics, physiology understanding, discharge planning expertise, utilization management skills, diagnostic evaluation proficiency, and familiarity with medical terminology. Experience in inpatient or outpatient settings is highly valued for this role.
