Sign-On Bonus Sign-On Bonus Available for qualified candidates Bonus details will be discussed during the interview process Position Summary The Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their homes in accordance with physician orders and agency policies. The RN will assess patient needs, develop care plans, and provide clinical oversight to ensure high-quality care. Responsibilities Perform initial and ongoing patient assessments Develop and update individualized care plans Provide skilled nursing services in the home Administer medications and treatments as ordered Monitor patient progress and document visits accurately Educate patients and families on care needs and safety Coordinate care with physicians and other healthcare providers Supervise Home Health Aides (HHAs) when required Complete documentation in a timely manner Participate in case conferences and team meetings Maintain compliance with agency policies and state regulations Qualifications Active Registered Nurse (RN) License Current CPR certification Valid driver's license and reliable transportation Ability to travel to patient homes Strong communication and organizational skills Home health experience preferred but not required Ability to work independently Work Environment Patient homes and community settings Flexible scheduling options available Supportive office staff and clinical leadership Combination of field work and documentation Independent work with team support
