Job Description

IND123 JOB DESCRIPTION
The Hospice RN is a key member of the Amberwell Hospice/Health at Home Team. The Hospice RN will work under the direction of the Director of Health at Home. The Hospice RN position will be responsible for the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation. For this role, you need to be an RN who sees the value of hospice care in alleviating pain and suffering. You should be a compassionate person with excellent interpersonal skills. We expect you to be knowledgeable in caring for the terminally ill and their family/caregivers. Duties may include but are not limited to: Able to work as a member of a team and independently. Understand and follow safe work practices. Ensure that all Amberwell procedures are followed in accordance with established policies. Collaborate with physicians and other professionals to plan care. Make frequent visits to the patient to evaluate their condition. Provide support and comfort according to individual patient's needs. Work with caregivers to plan and execute excellent patient care. Help in administering medication. Alleviate sudden symptoms of the disease's end phase. Notify a doctor about a patient's condition when deemed necessary. Assist patients and their families with paperwork. Maintain accurate and timely records per policy requirements. Help family/caregivers cope with the upcoming loss and provide post-mortem support.
