Education:
Associates Degree of Nursing. Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE). Possess one of the following certifications: Commission for Case Manager Certification Certified Case Manager (CCM)
Certification of Disability Management Specialists Commission:
Certified Disability Management Specialist (CDMS)
Association of Rehabilitation Nurses:
Certified Rehabilitation Registered Nurse (CRRN) American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S). National Board for Certification in
Continuity of Care:

Experience :
HCW must have at least 36 months of nursing experience with levels of care to include pre-hospital and post-hospital and acute care illness stabilization. Must have experience providing case management services for 24 recent consecutive months of direct patient specialty care experience. Working knowledge of the military health care system, including TRICARE health benefits, is preferred. The nurse must document clients' case management plans and ongoing activities and verifies that interventions meet the client's needs and goals of treatment. Knowledge of a wide range of medical disorders/conditions and disease processes across the lifespan, as appropriate. Must possess a working knowledge of working with interdisciplinary care teams and technology to identify high-risk, complex patient cases; manages care, including referral management, care planning, post-discharge planning, and coordinating community-based and transitional care. The nurse has knowledge of screening clients and/or populations for healthcare needs. Must possess a working knowledge of conducting individualized clinical evaluations of patients, their health needs, and concerns;
License:
Current, full, active, and unrestricted license to practice as a Registered Nurse
SPECIFIC TASKS.
The duties for the HCW are as follows but not limited to: As a member of the Medical Management team, the Registered Nurse Case Manager provides clinical case management, care coordination for patients enrolled at Military Treatment Facility. Monitors and facilitate adherence to treatments, provides education, coordinates care, maintains case management data registries, and performs full scope of clinical nursing in the execution of case management services. May be required to augment medical management nursing support during the absence of other team nurses due to illness, leave or deployment.
