This role is ideal for an independent RN who is passionate about providing quality end-of-life care and collaborating with an interdisciplinary care team. Position Summary The RN Case Manager will coordinate and manage hospice patient care, ensuring patients and families receive compassionate, individualized support throughout their care journey. Responsibilities include admissions, assessments, triage support, care coordination, death visits, and related clinical documentation. This is an on-call weekend position in which the RN will remain at home while on-call but must be available to respond to patient needs, including admissions, deaths, and triage calls, and report on-site as needed. Nurses are compensated for the full on-call shift. Responsibilities Conduct patient admissions and comprehensive nursing assessments Respond to hospice triage calls and patient care needs Perform death visits and provide compassionate family support Develop and coordinate individualized care plans Collaborate with interdisciplinary teams to support patient and family needs Complete accurate and timely clinical documentation Maintain compliance with clinical standards and company policies Qualifications Active Registered Nurse (RN) license in the State of Kansas required Minimum 1 year of RN experience preferred Hospice or Home Health experience preferred Strong written and verbal communication skills Ability to work independently and manage patient care effectively Reliable transportation and ability to travel locally Clean background check, driving record, and drug screen required Schedule Weekend On-Call Schedule Friday Night through Monday Morning RN remains on-call from home but must be able to report on-site as needed Full shift compensation provided during on-call coverage #IND3
