Hospice Homecare RN Case Manager
  • Topeka, KS | Full-Time | Days (8am-5pm) | Flexible self-scheduling, Occasional on-call Hospice Homecare RN Case Manager
  • Lawrence, KS | Full-Time | Days (8am-5pm) | Flexible self-scheduling, Occasional on-call Hospice Admissions RN
  • Topeka, KS | Full-Time | Days (8am-5pm) | Occasional on-call Inpatient Hospice RN
  • Topeka, KS | Full-Time | Days (7am-7pm, 36 hrs/week) Inpatient Hospice RN
  • Topeka, KS | Part-Time | Nights (7pm-7am, 24 hrs/week)
Job Duties:
Provide compassionate nursing care to hospice patients and their families Assess patient needs and develop individualized care plans Coordinate care with physicians and interdisciplinary team members Manage symptoms and support comfort and quality of life Educate patients and families regarding care plans and services Complete accurate and timely clinical documentation Support patients and families through end-of-life care with dignity and compassion
Requirements:
Active RN license Graduate of an accredited nursing program CPR certification Valid driver's license and reliable transportation Hospice, palliative care, acute care, home health, or case management experience preferred but willing to train
