Registered Nurse/MDS Coordinator Location:
Henderson, KY 42420 - Redbanks is a locally managed, community focused, not-for-profit skilled nursing center located in Henderson, Ky. Job Summary We are seeking a dedicated and detail-oriented Registered Nurse/MDS Coordinator to join our healthcare team in Henderson, KY. The ideal candidate will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments to ensure accurate and timely documentation of resident care and compliance with regulatory requirements. Key Responsibilities Conduct comprehensive MDS assessments for residents in accordance with federal and state guidelines. Coordinate with interdisciplinary team members to gather necessary clinical information. Ensure accuracy and completeness of MDS documentation to support quality care and reimbursement. Monitor resident care plans and update as necessary based on assessment findings. Assist in the development and implementation of care plans tailored to individual resident needs. Maintain compliance with all regulatory requirements related to MDS and resident care documentation. Provide education and training to staff regarding MDS processes and documentation standards. Collaborate with nursing staff, physicians, and other healthcare professionals to optimize resident outcomes. Participate in quality improvement initiatives and audits related to MDS and resident care. Qualifications Current and valid Registered Nurse (RN) license in the state of Kentucky. Minimum of 2 years nursing experience, preferably in long-term care or skilled nursing facility. Experience with MDS assessments and knowledge of CMS regulations preferred. Strong organizational and communication skills. Ability to work independently and as part of a multidisciplinary team. Proficient in electronic health records and MDS software systems. Work Environment This position is based in a skilled nursing facility in Henderson, KY. The role requires frequent interaction with residents, families, and healthcare team members in a fast-paced clinical environment.
