Pay:

$51.00 - $55.75 per hour based on experience

Type:

Three-month contract

Schedule:

36 hours guaranteed, 8:00AM - 4:30 (Monday - Friday)

On Call Requirement:

One weekend per month

Zip codes 40214, 40215, 40216, 40258, 40272, 40118

Primary Responsibilities:

• RN case manager for a specific territory
• Performing skilled routine visits, recerts, discharges and start of cares/resumption of cares.
• Perform SOCs, ROCs
• 100% patient care
• IV Care
• OASIS visits, wound care, picc line care

Qualifications:

• Current RN License in Kentucky in good standing
• Valid Driver's License
• CPR Certification
• Experience in homecare
• Experience in HCHB
