Job Description
MDS Coordinator
The Homeplace at Midway
Midway, KY MDS Coordinator
(MDS Long term care experience required)
Promotes the utilization of the Resident Assessment Instrument (RAI) to provide a structured, standardized approach for identification of a resident's strengths and needs which can then be addressed in an individualized plan of care.
Completes assessment process within regulatory guidelines and facility policy and procedures:
Completes Minimum Data Set (MDS) timely and accurately
Completes Care Area Assessments (CAAs) ensuring CAAs areprehensive andpleted on time
Enters data intoputer
Transmits data timely
Attaches all validating data
Developsprehensive care plan based on resident assessment
Completes individualized care plan within time frames
Includes all necessary Triggers in care plan
Monitors care plans for updates related to new and discontinued treatments
Coordinates care plan team
Sets schedule for RAI process
Utilizes therapy and other treatments to set the correct ARD date to maximize reimbursement
Participates in RUGS huddles to ensure correct data and maximizing of reimbursement
Leads care plan team and ensures all disciplines are inpliance with timeframes
Reviews case mix reports for accuracy and works with the state surveyors if t are problems
Analyzes the RUG category before sealing
Educates certified nurse aides about correct coding and documentation of Activities of Daily Living (ADL) and other MDS items for accurate care tracker entry
Communicates with the administrator, director of nursing, and unit managers regarding MDS and other issues as necessary
Keeps current with changes to the RAI and regulations
Initiates and participates in quality assurance activities to monitor Quality Indicators (QIs) and to carry out quality improvement activities.
Conducts training and monitors all staff involved in the MDS process to assure facility-widepliance.
Qualifications:
Required Education:
Associates degree in nursing from accredited college or universityRequired Experience:
2 years' in long term care with working knowledge of all long term care regulatory requirements; MDS 3.0, Resident Assessment Instrument (RAI) and Care Area Assessments (CAAs) process Required License and/orCertification:
Licensed as RN or LPN in Commonwealth of KentuckyBenefits:
