Job Description

Family Health Care Associates has been servicing communities across Kentucky since 2002. Our mission is to provide quality health care to all people in the communities we serve. 8am to 5pm Education and Experience Certified LPN required, RN preferred Current licensure is in good standing in the state of practice, and all other states where license is held 1-2 years of experience as a nurse Special Skills, knowledge and abilities Medical terminology Clinical skills: Proficiency in taking vital signs, drawing blood, administering injections, and assisting with exams Time management and adaptability skills is required Ability to operate standard office equipment License or Certification necessary: Certified LPN or RN CPR and/or BLS Certification required Physical Demands Nurses may spend a great deal of their professional day walking, standing, moving, and manipulating equipment, as well as lifting and moving objects, equipment, and supplies. It is reasonable to anticipate lifting 20-25 pounds in any given day. Lifting up to 40-50 pounds may occasionally be required, depending on the specific style or type of care provided. In the performance of certain patient care tasks, Nurses may be expected to have sufficient manual dexterity to safely perform various skills and techniques. Tasks and expectations may vary by clinical site and medical practice. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to stand and talk or hear. The employee is frequently required to use hands to finger, handle, or feel; and reach forward with hands and arms. The employee is required to sit and stoop, kneel, or crouch. Physical Demand Ratings are an estimate of the overall strength requirements that are important for an average, successful work performance of a specific job. To classify the occupation of a worker with a physical demand rating, the most strenuous and frequently performed tasks are evaluated. The overall physical demand rating for a job of the nurses falls within the Light classification according to the Dictionary of Occupational Titles. Position Summary The Nurse Coordinator is responsible for managing the individualized patient care by providing high-quality, culturally competent, and compassionate care while adhering to facility protocols and maintaining professional development. This nurse is responsible for promoting and restoring patients' health through the nursing process; collaborating with practitioners and multidisciplinary team members; and providing physical and psychological support to patients, friends, and families. In addition to the nurse, the Nurse Coordinator will also act as the vaccine coordinator for the area. Vaccine coordinators play an integral role in the vaccine cold chain. In addition to their regular job duties, the vaccine coordinator is responsible for ensuring all vaccines in their clinic are stored and handled correctly.
Position Responsibilities Assessment & Care Prioritization:
Conduct individualized patient assessments, prioritizing based on immediate needs, and adjust care as the patient's condition evolves.
Care Planning & Collaboration:
Develop and update individualized care plans in collaboration with the healthcare team.
Treatment & Interventions:
Administer treatments and perform therapeutic nursing interventions as per the care plan and practitioner orders.
Vaccine Coordinator:
Act as the vaccine coordinator by ensuring all of the tasks required of vaccine storage and handling, such as vaccine oversight, vaccine storage, temperature monitoring, and inventory management. (See the complete list of duties for Kentucky Public Health attached).
Cultural & Age Competence:
Deliver care that is sensitive to the patient's age, culture, and specific needs.
Patient & Family Education:
Provide customized education to patients and families regarding health and care.
Documentation:
Accurately document assessments, responses to interventions, and progress.
Emergency Response:
Initiate emergency resuscitation and life support measures when required.
Confidentiality & Professionalism:
Maintain patient confidentiality and deliver care in a non-judgmental, non-discriminatory manner that respects the patient's rights.
Communication & Reporting:
Report patient condition to appropriate personnel at the start and end of shifts.
Continuing Education & Competency:
Stay current in nursing practices through ongoing education, workshops, and professional engagement.
Documentation Proficiency:
Be proficient in both paper-based and electronic medical record documentation. Other roles/duties will be assigned as necessary to assist FHCA in the attainment of the goals set forth and the enhancement of a positive, respectful family practice for all patient and employees.
