Job Description

Overview Join our compassionate healthcare team as an On-Call Hospice Nurse, where your expertise will provide vital comfort and care to patients nearing the end of life. This dynamic role offers flexibility and the opportunity to make a meaningful difference in patients' lives during their most vulnerable moments. As an On-Call Hospice Nurse, you will deliver specialized nursing care, coordinate with interdisciplinary teams, and respond promptly to patient needs in diverse settings, including patients' homes and healthcare facilities. Your critical care experience and clinical skills will be essential in managing complex cases with empathy and professionalism. Duties Provide comprehensive nursing assessments and symptom management for hospice patients in accordance with care plans. Respond swiftly to on-call emergencies, including urgent patient needs related to pain control, medication adjustments, or crisis intervention. Collaborate with physicians, social workers, chaplains, and other team members to ensure holistic patient care. Educate patients and families about disease progression, medication administration, and comfort measures to support informed decision-making. Document all patient interactions accurately in medical records while adhering to HIPAA regulations and confidentiality standards. Coordinate discharge planning and facilitate transitions between inpatient, outpatient, or post-acute care settings as needed. Conduct vital signs assessments, monitor physiological changes, and evaluate patient responses to interventions during on-call shifts. Qualifications 2years experience as a RN Experience in hospital medicine, medical-surgical units, or post-acute care environments such as skilled nursing facilities or outpatient clinics. Strong knowledge of physiology, medical terminology, infection control practices, and diagnostic evaluation procedures. Ability to perform patient assessments accurately and respond effectively under pressure during urgent situations. Prior experience working with diverse populations including geriatrics and pediatrics is highly desirable. Valid nursing license with current certification; additional certifications in emergency or trauma nursing are a plus. $73,043.71 - $87,966.62 per year
