LIVINGSTON PARISH AND SURROUNDING AREA NURSES - HOSPICE
Field Nurse (Full-Time)
WEST BATON ROUGE AND SURROUNDING AREAS - HOSPICE
Field (Full-Time) Audubon Hospice is growing and currently seeking a Full-time Hospice RN Field Case Manager, who shares our mission and commitment to providing compassionate, high-quality end of life care to patients and families in Livingston Parish, West Baton Rouge and surrounding areas. Come join our staff of experienced, knowledgeable team members willing to help you in any way needed. Requirements include, but are not limited to:
  • Strong clinical, problem solving, and critical thinking skills with sound clinical judgement; able to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others.
  • Initiative-taking possesses effective organization and time management skills; able to work both independently and in coordination with the interdisciplinary team.
  • Unencumbered LA RN license
  • Valid Driver's License with a clean driving record and reliable transportation with valid proof of insurance
  • Must be able to pass background check and drug screen
  • Technology and typing proficiency to enter/retrieve data in electronic clinical records, experience with email, internet research, and other software applications. Knowledge of WellSky EMR a plus.
  • Effective and timely documentation skills to maintain compliance with agency and regulatory guidelines.
  • Knowledge of and adherence to Regulatory Guidelines and agency Policy and Procedures. Responsibilities include, but are not limited to:
  • Develops an initial and ongoing individualized Plan of Care in collaboration with the patient, the family/caregivers, and Interdisciplinary Team. Implements the Hospice Plan of Care with ongoing evaluation to ensure interventions are appropriate and effective to meet patient and family/caregiver needs at each stage in the disease process.
  • Promotes understanding of Hospice Philosophy Conducts routine visits, admissions visits, death visits, and/or follow-up visits in both home and facility settings.
  • Attends and participates in morning stand up conference calls, weekly staffing meetings, and all interdisciplinary group meetings.
  • Continuing education; maintains professional licensure and credentials; represents the agency in a professional, ethical manner.
