Urgently hiring Experienced RN/QA Nurse

Quality Assurance & Compliance Conduct routine chart audits for accuracy, completeness, and regulatory compliance. Monitor documentation related to nursing services, treatments, physician orders, incidents, and care plans. Assist with Quality Assurance Performance Improvement (QAPI) programs and meetings. Track and trend clinical indicators including falls, infections, wounds, weight loss, hospital readmissions, and medication errors. Ensure compliance with CMS, state regulations, and facility policies. Prepare for and assist during state surveys and inspections. Follow up on corrective action plans and monitor outcomes. Clinical Responsibilities Review resident assessments and care plans for accuracy and appropriateness. Monitor residents for changes in condition and communicate concerns to physicians and nursing leadership. Assist with admissions, transfers, and discharges as needed. Ensure proper infection control practices are followed throughout the facility. Participate in wound management and skin integrity monitoring when assigned. Assist nursing staff with clinical guidance and support. Documentation & PointClickCare Responsibilities Audit PointClickCare documentation daily for accuracy and completion. Monitor skilled documentation, physician orders, TAR/MAR completion, and nursing charting. Ensure timely completion of assessments and documentation requirements in PointClickCare. Generate and review QA reports, clinical dashboards, and compliance tracking reports. Assist staff with PointClickCare documentation training and troubleshooting. Staff Education & Training Educate staff on policies, procedures, documentation standards, and regulatory requirements. Conduct or assist with nursing in-services and competency evaluations. Promote adherence to infection control, safety, and resident rights standards. Assist with orientation of new nursing employees. Incident & Risk Management Investigate incidents, accidents, and resident grievances. Complete follow-up documentation and recommendations for prevention. Monitor trends and recommend interventions to reduce risks and improve resident outcomes. Qualifications Current and active Registered Nurse (RN) license in the State of Louisiana. Minimum of 1-2 years nursing experience in long-term care preferred. Knowledge of CMS regulations, state survey process, and nursing standards. Experience with PointClickCare preferred. Strong organizational, communication, and leadership skills. Ability to maintain confidentiality and professionalism. Physical Requirements Ability to stand, walk, bend, lift, and assist residents as necessary. Ability to lift up to 50 pounds occasionally. Ability to work flexible hours as needed. Work Environment Long-term care/skilled nursing facility environment. Exposure to infectious diseases, bodily fluids, and medical equipment.
