Job Description
We are looking for a Registered Nurse to join our team and provide compassionate care to patients in hospice. The ideal candidate will have a passion for helping others and a strong commitment to providing quality care.
Responsibilities:
Identify the patient/family's physical, psychosocial and environmental needs and reassess as needed. Provide nursing services in accordance with the plan of care. Document issues, appropriate goals, interventions, and patient/family response to hospice care. Collaborate with the patient/family attending physician and the other members of the interdisciplinary team in providing patient and family care. Complete and submit accurate clinical notes regarding the patient's condition into patient's chart. Demonstrates competency in skills required by the agency for patient care. Collaborates with IDT team significant findings, problems, changes in condition. Initiates, updates as needed, and adheres to the patient's plan of care to maintain continuity of patient care by communication with appropriate staff. Completes admissions and makes follow-up visits to patients admitted the day prior; as necessary. Administers medications and treatments in accordance with the patient's plan of care. Works in cooperation with the family/caregiver and IDT to meet the emotional needs of the patient/family/caregiver. Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains physician orders as required. Recognizes standards of practice to achieve excellent pain and symptom management and high-quality end-of-life care. Performs other duties as assigned. Supervises the care given by LPN nurses during on call shift. Operates within the nursing scope of practice by following the state and federal regulations for hospice. Strong communication skills are a must.Qualifications required:
Two (2+) years of nursing experience in hospice Registered Nursing license in the state of Louisiana Current CPR certification Able to provide emergency care as needed for any patient. Critical thinking skills is a must. Timely charting in EMR and knowledge of our EMR is required. Previous hospice or home health experience necessary. Maintains a neat, well-groomed appearance at all times. Keeps personal appearance appropriate for patient care.Job Type:
