As a Hospice Admissions Nurse, you will be responsible for executing the hospice admission process in compliance with federal and state regulations. You will advocate for the patient and family to meet their needs that hospice can cover and refer them to other resources as appropriate.

Primary Responsibilities:

Demonstrates competency with potential patients, hospice eligibility and meeting with families in assessment of their needs Maintains confidentiality and safety of patient/client clinical records Completes assigned admissions and patient care orders in an effective and timely manner Communicates effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding the new admission of a patient. Records that may be relevant to determine hospice eligibility Communicates effectively with operational, clinical and sales leadership regarding status of pending referrals Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.

Required Qualifications:

Current unrestricted RN licensed in the state of operation Current Driver's License and vehicle insurance, and access to a dependable vehicle or public transportation

State Specific Requirements:

LA:

a minimum of two years full-time experience working as an RN. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse.

Preferred Qualifications:

Current CPR certification or ability to complete within 90 days of hire 2+ years of experience in a hospice, home health, or other healthcare service delivery system setting Demonstrated fiscal responsibility and accountability Proficient computer skills with knowledge of Microsoft Word and Excel Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
