Job Description

Job Overview We are seeking a dedicated and compassionate Home Health Field Registered Nurse (RN) PRN to serve the Lower River Parishes area. This role involves providing high-quality, patient-centered nursing care in the home setting, supporting patients through recovery, chronic disease management, and post-acute care. The ideal candidate will possess extensive clinical experience across various specialties, including critical care, emergency medicine, and hospital-based nursing, ensuring comprehensive assessments and personalized care plans. As a vital member of our home health team, you will promote health, safety, and independence for diverse patient populations while adhering to all regulatory standards. Duties Conduct thorough patient assessments in the home environment, including vital signs, physical examinations, and review of medical history. Develop and implement individualized care plans aligned with physician orders and patient needs. Manage complex cases involving critical care, infectious diseases, geriatrics, behavioral health, or post-acute conditions. Provide patient education on medication management, disease process, wound care, and health promotion strategies. Coordinate with interdisciplinary teams to facilitate discharge planning and ensure seamless continuity of care. Document all patient interactions accurately within electronic health records (EHR) systems while maintaining HIPAA compliance. Monitor patient progress through regular evaluations and modify care plans as necessary. Assist with managing patients as a nurse case manager by overseeing treatment adherence and facilitating communication among providers. Support outpatient or clinic-based services by managing intake assessments and diagnostic evaluations. Educate patients and families on health regulation policies to promote safety and compliance. Requirements Valid registered nurse (RN) license in good standing with relevant state licensing boards. Proven clinical experience in critical care settings such as ICU or Level I Trauma Centers. Experience in hospital medicine, emergency medicine, or post-acute care environments. Knowledge of medical terminology, physiology, and medical records management. Familiarity with electronic health records (EHR) systems and documentation standards. Strong assessment skills with the ability to evaluate complex patient conditions effectively. Understanding of HIPAA regulations and patient privacy protocols. Ability to manage multiple cases efficiently while maintaining high-quality patient service. Experience with discharge planning, utilization management, and diagnostic evaluation processes. Competency in managing patients across diverse populations including pediatrics, geriatrics, or behavioral health.
