Job Description
Hospice Nurse Metairie, LA Part-time, Full-time Part-time, Full-time
- We are looking for a Full time Nurse to service our patients in Metairie and New Orleans area for our Transitional Care Program to bridge patients between Home Health & Hospice.
Duties:
- Provide direct and indirect patient care in a variety of healthcare settings
- Perform assessments, develop care plans, and implement nursing interventions
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and report any changes or abnormalities
- Provide education and support to patients and their families on various healthcare topics
- Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
- Document patient care activities accurately and timely
Experience:
- Minimum of 2 years of experience as a licensed Nurse
- Experience in Hospice Care and or Home Health
- Proficient in planning and coordination of care for patients transitioning to home or other healthcare facilities
- Knowledge of Durable Medical Equipment (DME) and its usage in patient care
Requirements:
- Valid Registered Nurse license in LA
- Current CPR certification
- Strong communication and interpersonal skills
- Ability to prioritize and multitask in a fast-paced environment
