Work when you want No set schedule- ideal for supplemental income What You'll Do Lead Patient-Centered Hospice Care Perform comprehensive patient and family assessments Develop and manage individualized, comfort-focused care plans Deliver skilled nursing care with a focus on pain and symptom management Coordinate Care Across the Team Serve as case manager across the interdisciplinary team Collaborate with physicians, social workers, chaplains, and aides Monitor patient condition and adjust care to maintain comfort Support Patients & Families Provide education, emotional support, and guidance Help families navigate complex decisions with empathy and clarity Build meaningful, trusting relationships Ensure Quality & Compliance Maintain timely, accurate documentation Participate in IDT meetings and care planning Support quality and compliance standards What You Bring Active RN license (or compact) + CPR Graduate of an accredited nursing program 1+ year of RN experience Strong clinical judgment with a compassionate approach Ability to manage a caseload independently Excellent communication skills in sensitive situations EMR proficiency Valid driver's license and reliable transportation Nice to Have Hospice, palliative, or home health experience Strong pain and symptom management skills Case management experience Experience with Homecare Homebase (HCHB) Territory Metairie, LA and surrounding areas Benefits That Support You (FT Team Members) Health & Wellness Medical, Dental, Vision Pharmacy + Virtual & Mental Health Support
