Job Description
Job Summary We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) Case Manager to join our team and provide exceptional end-of-life care to patients and their families. In this vital role, you will coordinate comprehensive patient care plans, assess patient needs, and deliver personalized support to ensure comfort, dignity, and quality of life. Duties Conduct thorough patient assessments, including vital signs, physiological evaluations, and medical history reviews to determine eligibility for hospice services. Develop and implement individualized care plans in collaboration with interdisciplinary teams, ensuring alignment with patient wishes and clinical needs. Provide direct patient care within various settings such as inpatient units, outpatient clinics, or post-acute care facilities, emphasizing symptom management and comfort measures. Coordinate discharge planning and transition of care to optimize patient outcomes across hospital, outpatient, and community settings. Manage utilization review and case management activities by evaluating medical records, diagnostic evaluations, and treatment progress to support appropriate resource utilization. Educate patients and families about disease processes, medication management, advance directives, and available support services while ensuring compliance with HIPAA regulations. Maintain accurate documentation of patient interactions, medical records, and case notes in accordance with hospital policies and regulatory standards. Experience Active Registered Nurse (RN) license in good standing Minimum of 1-2 years of nursing experience (hospice, home health, or acute care preferred) Experience in hospice or palliative care strongly preferred Skilled in comprehensive patient assessments and care planning Experience coordinating care with an interdisciplinary team (IDT) Knowledge of end-of-life care practices , symptom management, and comfort-focused interventions Experience with medication management , including pain control and symptom relief Familiarity with Medicare/Medicaid hospice regulations and documentation requirements Ability to develop, implement, and update plans of care Strong documentation skills using electronic medical records (EMR) systems Experience providing patient and family education on disease progression and care needs Ability to manage a caseload independently and prioritize effectively Experience collaborating with physicians, social workers, chaplains, and other disciplines Strong communication and interpersonal skills, especially in emotionally sensitive situations Experience with on-call rotation and responding to urgent patient needs (preferred) Valid driver's license and ability to travel to patient homes
