Job Description

Guardian Home Healthcare of LA is locally owned and operated full-service home health agency that has been in business and growing for over 24 years. We are a Medicare certified agency and also own My Hospice. We pride ourselves in providing outstanding, high quality healthcare to each of our patients with the time and attention necessary to meet his or her needs. Qualifications
  • Active Louisiana RN License
  • Home Health experience preferred but not necessary, minimum one year hospital experience required per State law
  • Have a reliable vehicle with active driver's license and insurance
  • Current CPR certification If No Home Health Experience Most of our Registered Nurses over the years have been hired from hospitals. You can make 25-35% higher pay while not having to work nights. You must have a minimum of 1-year experience and strong nursing skills. You must also enjoy teaching patients concerning their disease processes and medications. Requirements and Responsibilities
  • Provides an initial, resumption, and re-certification assessment of the Home Health patient in their home setting.
  • Coordinates the initial disciplines to begin care, including the planning of care throughout the patient episode, collaborating with the physician, patient, family member and team manager to develop the expected patient visits.
  • Documents care for home health patients from day of admission through day of discharge
  • Communicates changes in the condition and needs of the patient with all team members including Physicians, Nurses, Therapists, Social Workers, Home Health Aides, patient, family, and other individuals to ensure that the patient's plan of care is appropriately developed.
  • Understands and adheres to established Agency employee handbook, policies and procedures
