Competitive pay, Benefits, Paid by experience MDS Coordinator job duties Completing and updating MDS forms on all patients in the care facility Performing clinical assessments to gather information on residents Developing care plans for patients to coordinate available services Submitting updates and paperwork to maintain compliance Participating in the admission and discharge processes Conducting and attending meetings with other care team members Educating patients and their families Understanding of Medicare and Medicaid requirements Proficient with the MDS assessment tool, including MDS coding knowledge An understanding of medical terminology, nursing practices and patient care standards Attention to detail to ensure precision in completing assessments and accurate data entry Assessment, data collection and documentation skills Written and verbal communication skills Organizational skills Comfort with use of PCC Point Click Care electronic records
