Essential Responsibilities:
Provides each resident with professional nursing care, assessments and evaluations. Responsible for the management of nursing activities performed by Neighborhood/Household (NH/HH) staff ensuring that the highest degree of quality care is maintained at all times Works with a team of cross-trained staff to coordinate the functions of the NH/HH Has 24 hour responsibility for clinical functions of the NH/HH Qualifications Perform initial, routine, and urgent health assessments (including head-to-toe physical exams, vital signs, and mental status evaluations). Gather and analyze detailed patient medical histories, current symptoms, medication regimens, and psychosocial needs. Formulate precise nursing diagnoses and collaborate with physicians, therapists, and social workers to develop tailored, evidence-based patient care plans. Utilize clinical judgment to flag critical risks, such as fall hazards (e.g., Morse Fall Scale), cognitive decline, nutritional deficiencies, or sudden changes in status. Maintain highly accurate, timely electronic health records (EHR) in strict compliance with state, federal, and institutional regulations. Educate patients and their families regarding health statuses, care choices, rights, and upcoming discharge plans. Oversee the federally mandated Minimum Data Set (MDS) process, ensuring precise coding for reimbursement and regulatory compliance. Other Qualifications Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) required. Active, unrestricted Registered Nurse (RN) license in the state of employment. Current Basic Life Support (BLS) / CPR certification. Minimum of 2-3 years of clinical nursing experience (Home Health, or Geriatrics preferred). Prior experience with MDS, OASIS, or utilization review is a major plus. Exceptional physical assessment and diagnostic reasoning skills. Ability to ask sensitive questions comfortably, actively listen, and translate complex medical jargon into plain language for families. Proficiency with Electronic Health Record (EHR) systems and standard data entry tools. High level of autonomy with strong time-management skills to balance multiple complex assessments daily.
