Home Care RN Case Manager

Community Health Network has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager. Coverage needs in the Arlington, MA area. Monday-Friday, 40 hours/week (Part-time hours considered) No weekends, holidays or on-call required Consolidated territory Competitive pay rates + IRS mileage reimbursement 13 week contracts The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides. Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
