- $3120/weekly Duties & Responsibilities Conduct comprehensive patient assessments to identify clinical, psychosocial, and discharge needs.
Develop, implement, and monitor individualized patient care plans. Coordinate care across multidisciplinary teams, including physicians, nurses, therapists, and social workers. Facilitate safe and timely discharge planning and transitions of care. Ensure appropriate utilization of healthcare resources and services. Educate patients and families regarding diagnoses, treatment plans, medications, and community resources. Advocate for patients to ensure high-quality, patient-centered care. Monitor patient progress and adjust care plans as needed. Collaborate with insurance providers regarding authorizations and medical necessity when applicable. Maintain accurate and timely documentation in the electronic health record (EHR). Identify barriers to care and coordinate appropriate interventions. Ensure compliance with hospital policies, regulatory standards, and case management guidelines. Participate in interdisciplinary rounds and care conferences. Coordinate referrals to post-acute services, including home health, rehabilitation, skilled nursing facilities, and hospice as appropriate. Promote quality outcomes, patient satisfaction, and cost-effective care throughout the continuum of care. Requirements Active RN license. 3-5 years of recent RN Case Management experience. Acute care/hospital experience required. Experience with care coordination, discharge planning, and utilization review. Proficiency with Epic system BLS certification required
