Hospice Registered Nurse (RN) Case Manager - Greater Boston Area Join a team that leads with heart, skill and a commitment to purpose We are seeking a compassionate and detail oriented Registered Nurse (RN) Case Manager to join our experienced hospice team serving patients and families throughout the greater Boston and surrounding areas. As part of a highly supportive group of seasoned hospice clinicians, you will have the autonomy to manage your own caseload while enjoying the camaraderie, respect and collaboration of a team that truly works together. What you'll do: Provide comprehensive, compassionate care to patients and families in their homes or facilities. Coordinate interdisciplinary care and ensure timely communication among team members, patients and those who care for them. Advocate for patients' wishes while maintaining the highest standards of clinical excellence. Manage visit schedule, Medicare and organizational compliant documentation with attention to detail.
