Registered Nurse TotalCare - 3.1 Boston, MA Job Details Part-time | Per diem $35 - $45 an hour 16 hours ago Qualifications CPR Certification RN License Working with individuals with disabilities BLS Certification Senior care Care plans Nursing Full Job Description We are seeking a Registered Nurse to join our team! You will be responsible for the intake, assessment, diagnosis, care plan and treatment of assigned patients. The ideal candidate will be a responsible well-trained professional able to give the best nursing care with little supervision. You will be compassionate and cool-tempered. You will be able to follow masshealth regulations. The goal is to promote patient's well-being by providing high quality nursing care.
Responsibilities:
Evaluation of potential members; Contact with members primary care physician to obtain medical information and physician clearance; Completion of the medical components of the member's assessment forms; Monitoring of the members health status and completion of semi-annual member health status report and patient care plan Visits patient on regular basis. Review and supervision of personal care services at least every sixty (30-60) days. Educate all staff on related health issues Communicates health condition of member to the MDT Team Implements established policies and procedures Record observations and functioning status of member Planning, implementing, and evaluation of foster care setting Provision of case management
Completion of MDS Qualifications:
Current Massachusetts licensure as Registered Nurse with Two years experience working with disabled/elderly adults in a community setting.
Summary:
The Registered Nurse evaluates and assesses the general health functioning of all GAFC, AFC, HHA and PCP members and designs and revises the care plan for each member. The Registered Nurse, Case Manager and Administrative staff work together as a multidisciplinary team to ensure an effective and quality oriented program for all members.
