  • 4:00 PM (Weekday, Weekend & Holiday Rotation)
Job Overview:
We are seeking a compassionate and experienced Travel Registered Nurse (RN)
  • Hospice to provide high-quality end-of-life care to patients in home and facility settings.
This role requires strong clinical judgment, effective communication, and the ability to manage a patient caseload independently while collaborating with an interdisciplinary team.
Key Responsibilities:
Provide compassionate hospice nursing care to patients in homes and care facilities Conduct comprehensive patient assessments and respond to changes in condition Manage and coordinate hospice patient caseloads and individualized care plans Participate in interdisciplinary team meetings and case conferences Communicate effectively with physicians, patients, caregivers, and healthcare teams Complete patient admissions, charting, and documentation accurately and on time Obtain and clarify physician orders for patient care Educate patients and families on hospice philosophy, care plans, and services Ensure compliance with hospice regulations, nursing standards, and internal policies Supervise Hospice Aides every 14 days per regulatory guidelines Support orientation and mentoring of new staff as needed
Required Qualifications:
Active Registered Nurse (RN) license
  • Massachusetts Minimum 2 years of Hospice RN Case Management experience Strong clinical assessment and critical thinking skills Excellent verbal and written communication skills Ability to work independently and manage schedules efficiently Successful completion of pre-employment medication competency test
Preferred Qualifications:
Prior Hospice Home Care experience IV Certification BSN (Bachelor of Science in Nursing) preferred Proficiency with electronic medical records (EMR) systems
Work Environment:
