Schedule:
Monday-Friday | 9:00 AM-5:00
PM Assignment:
13 weeks to start (Temp-to-Perm & Direct Placement options available)
Compensation Pay Rate:
Up to $57/hour
Mileage Reimbursement:
$0.72/mile Position Overview Amergis Healthcare Staffing is partnering with a highly respected Adult Day Health organization committed to helping older adults and individuals with disabilities remain safe, independent, and supported in their homes. Through individualized care plans, clinical oversight, caregiver education, and comprehensive case management, this organization delivers high-quality, community-focused care. As an RN Case Manager, you will play a key role in conducting clinical assessments, developing care plans, and ensuring continuity of care. You will collaborate with an interdisciplinary team to deliver proactive, patient-centered care. Key Responsibilities Clinical Assessments & Care Planning Conduct in-person intake, admission, annual, and reassessment visits in accordance with program requirements Perform comprehensive nursing assessments, evaluating physical, psychosocial, and environmental needs Collaborate with the interdisciplinary team to develop, implement, and update individualized care plans Assist with medication reviews and care plan updates Participate in discharge planning and ongoing care coordination Client & Caregiver Support Educate clients and caregivers on health maintenance, disease prevention, medication management, and available resources Review admission materials and ensure understanding of program expectations Provide caregiver training and education to support quality care delivery Assist clients and families in accessing healthcare services and community resources Ongoing Monitoring & Case Management Conduct regular in-home visits to monitor health status, caregiver performance, and environmental safety Address changes in physical health, psychosocial status, or living conditions Review caregiver documentation and communication tools Communicate effectively with clients, caregivers, providers, and healthcare professionals Participate in multidisciplinary team meetings Documentation & Compliance Complete all documentation accurately and in a timely manner at point of service Maintain detailed records of assessments, care plans, visits, and outcomes Ensure compliance with all regulatory and organizational standards Use company-provided tablets for all documentation Promote a culture of quality care, customer service, and compassion Represent organizational values with professionalism and integrity Perform other duties as assigned Qualifications Current Massachusetts RN license (in good standing) Minimum of 2 years of recent experience working with older adults or individuals with disabilities Valid driver's license, reliable transportation Ability and willingness to travel throughout assigned territory Strong communication, documentation, organization, and interpersonal skills Ability to work independently and manage multiple priorities effectively Strong critical thinking and problem-solving skills Preferred Qualifications Experience in case management, home health, community health, AFC/GAFC, behavioral health, or social services Experience coordinating care and navigating community resources Multilingual (English as primary or secondary language) Experience working with diverse, community-based populations Work Environment & Physical Requirements Frequent travel for in-home client visits Office work in a standard professional environment Ability to work in varying home conditions (temperature, pets, allergens, etc.) Prolonged computer use and documentation Ability to lift, carry, push, or pull up to 35 lbs Ability to walk, stand, sit for extended periods, and navigate stairs Requires visual acuity and manual dexterity Interested?
