Job Description

Braintree, MA, USA Home Health > Massachusetts > Clinical Operations > Skilled Nursing > Registered Nurse, Case Manager About the Role The Registered Nurse (RN) at Elmwood Healthcare - Home Health ( Elmwood Home Care ) will be responsible for delivering comprehensive nursing care to patients in their homes. You will perform a range of clinical tasks, coordinate care with other healthcare professionals, and provide education and support to patients and their families. The RN will work independently and collaboratively to ensure that each patient receives the highest standard of care tailored to their individual needs. Department Clinical Operations > Skilled Nursing Status / Type Full Time (40-Hours / Week) Schedule Monday through Friday; 8:00 A.M. to 4:30 P.M.;
On-Call Rotation Environment Field-Based Key Responsibilities:
Patient Care:
Provide direct nursing care, including administering medications, wound care, IV therapy, and other treatments as prescribed by the physician. Monitor and assess patients' conditions and adjust care plans as needed.
Assessment and Evaluation:
Conduct comprehensive assessments of patients' health status, including vital signs, medical history, and physical examinations. Develop and implement individualized care plans based on assessment findings and patient goals.
Care Coordination:
Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure a cohesive approach to patient management. Communicate effectively with the interdisciplinary team to address any changes in patients' conditions or needs.
Patient and Family Education:
Educate patients and their families on disease management, medication administration, self-care techniques, and lifestyle modifications. Provide guidance and support to help patients and families manage health conditions and enhance their quality of life.
Documentation:
Maintain accurate, detailed, and timely documentation of patient care, including progress notes, treatment plans, and any changes in patients' conditions. Ensure documentation complies with company policies, insurance requirements, and regulatory standards.
Health and Safety Compliance:
Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe care environment. Use medical equipment and supplies safely and appropriately.
Emotional Support:
Provide compassionate support to patients and their families, addressing concerns and offering comfort and reassurance as needed. Foster a positive and supportive relationship with patients to encourage adherence to care plans. Direct Patient Care /

Medication Management Education and Reconciliation Wound Care (Basic and Advanced, NPTW, etc.) Infusion Therapies (Excluding IV Push) Drain Care and Management Catheter Care and Management Ostomy Care and Management VAD Management and VAD Site Care (Excluding Discontinuation) Venipuncture and Blood Sample Collection via VAD
