The Homecare Nurse will report to the Nursing Supervisor and will play a vital role as part of the clinical team. The Primary role is to admit patients to the agency. This will include SOC , ROC, DC and may also do revisits as needed. Experience in homecare is preferred. The Homecare Admission Nurse ensures quality and safe delivery of nursing and home health services rendered. Coordinates provision of home health care services that reflect the agency philosophy and standards of home health and nursing care of assigned patients. Ensures quality and safe delivery of nursing care services.
Minimum Qualifications:
Education o Bachelor's or associate degree in Nursing Experience o Leadership skills and abilities adapt and learn quickly as well as to supervise and teach others Licensure and/or Certification o Current license to practice as a registered professional Nurse in Massachusetts Skills o Current knowledge of home care Nursing practices o Ability to respond to rapid changes in patient scheduling o Ability to work effectively as a team member o Excellent customer service skills relating to physicians, clinicians, patients and families o Excellent organizational and prioritizing skills o Computer literacy in Microsoft Word, Excel and Outlook o Excellent organizational skills o Must have the ability to read, write, and communicate in English o Ability to maintain confidential information and adhere to agency policies
