Day Shift Position Overview:
We are seeking a compassionate and skilled Registered Nurse (RN) to provide comprehensive home health care services to patients within the community setting. The RN will perform thorough nursing assessments, develop individualized care plans, coordinate interdisciplinary services, and ensure high-quality patient-centered care while promoting patient safety and independence. This role requires strong clinical judgment, excellent documentation skills, and the ability to work independently in a home care environment.
Key Responsibilities:
Patient Assessment & Care Planning Perform comprehensive nursing assessments upon patient admission including physiological, psychosocial, environmental, and health history evaluations Identify patient problems and establish individualized plans of care Evaluate patient progress and revise care plans as necessary Utilize HHRG values to determine visit frequency and interdisciplinary utilization Evaluate patient clinical needs and implement skilled nursing interventions Provide direct patient care and monitor patient responses to treatment Notify physicians and care providers of significant patient condition changes Coordinate patient discharges and complete discharge documentation appropriately Documentation & Coordination Complete admission and clinical documentation in a timely manner Maintain accurate, concise, and detailed narrative nursing notes Ensure documentation reflects skilled nursing services provided Obtain and clarify physician orders for all patient care activities Complete all required discharge paperwork and summaries Participate in interdisciplinary patient case conferences Collaborate with physicians, therapists, caregivers, and healthcare teams to ensure continuity of care Maintain compliance with agency policies, standards, and regulatory requirements
Required Qualifications:
Current Registered Nurse (RN) License in Massachusetts Current CPR Certification Successful completion of VNA pre-hire medication test Minimum 1 year of nursing experience required OR 6 months of nursing experience with
BSN Preferred Qualifications:
Bachelor of Science in Nursing (BSN) Home Health experience preferred IV Therapy experience preferred Strong computer and EMR documentation skills required For more details contact at or Call / Text at 516-862-1169.
