This role offers a balance of clinical care, client interaction, and caregiver support within a home care setting. Key Responsibilities Assist with initial client assessments and support development of care plans Conduct routine nursing visits to monitor client condition and home safety Perform re-assessments following hospitalizations or extended leaves Complete supervisory visits for caregivers (HHAs/CNAs) and provide guidance as needed Communicate care plan updates with caregivers and administrative staff Address client concerns and ensure timely follow-up Monitor care plan compliance and report any changes or concerns Maintain therapeutic relationships with clients and families Ensure adherence to infection control and universal precautions Document all visits and communications in WellSky in a timely manner Assist with monthly progress summaries as required Respond to client issues and support care coordination during acute situations Qualifications Active Licensed Practical Nurse (LPN) license in Massachusetts 1-2 years of clinical experience (home care experience preferred) Strong communication and organizational skills Ability to work independently in the field Familiarity with EMR systems (WellSky experience a plus) Reliable transportation
