Job Description

Hospice Services of Massachusetts is seeking a licensed and experienced RN Hospice Case Manager to join our team in the North Easton area. As an RN Hospice Case Manager, you will be responsible for supervising hospice aides, conducting comprehensive assessments of patients and families, and providing professional nursing care to patients in their homes and communities. You will also participate in revising the plan of care as necessary and use health assessment data to determine nursing diagnosis. $42 to $52 an hour with weekly pay. Salaried role.
Responsibilities:
Supervise hospice aides to ensure quality patient care Conduct initial, comprehensive, and ongoing assessments of patient and family needs Provide professional nursing care by utilizing all elements of the nursing process Develop and initiate plan of care, and make necessary revisions as patient status and needs change Use health assessment data to determine nursing diagnosis Develop a care plan that establishes goals and incorporates palliative nursing actions Initiate preventive and rehabilitative nursing procedures, and administer medications and treatments as prescribed Counsel patients and families in meeting nursing and related needs Provide health care instructions to patients as appropriate Assist patients with activities of daily living and facilitate self-sufficiency and optional comfort care
Requirements:
Graduate of an accredited school of nursing One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care Current RN license in Massachusetts and CPR certification Bachelor's degree, with one year of hospice care experience preferred What we offer: Competitive and Weekly Pay Holiday Pay for Hourly and Salaried Employees Overtime Pay for Hourly Employees Career Advancement Opportunities Mileage Reimbursement Exclusive Employer Discount Program Available for
