Job Description

Divine Living Homecare is seeking a compassionate and experienced Registered Nurse (RN) to provide clinical oversight and support for our growing home care agency. The RN will be responsible for client assessments, care planning, supervisory visits, caregiver training, quality assurance activities, and coordination of care for seniors, adults with disabilities, and individuals receiving services in home and community settings. This position offers flexible scheduling and the opportunity to make a meaningful impact while supporting clients in maintaining their independence and quality of life. Essential Duties and Responsibilities Conduct initial nursing assessments and ongoing reassessments. Develop, implement, and update individualized care plans. Complete supervisory visits and caregiver competency evaluations. Train, mentor, and provide clinical guidance to caregivers and direct care staff. Monitor client health status and communicate significant changes to appropriate healthcare providers and family members. Participate in admissions, service initiations, and case reviews. Ensure compliance with agency policies, regulatory requirements, and quality standards. Provide education to clients and families regarding health conditions, safety, wellness, and disease management. Assist with incident reporting, infection control, and quality assurance activities. Collaborate with healthcare professionals, hospitals, rehabilitation facilities, social workers, case managers, and community providers to coordinate services. Maintain accurate, timely, and professional documentation. Support care coordination and continuity of services for clients receiving home-based care. Participate in on-call nursing support as needed. Qualifications Current Massachusetts Registered Nurse (RN) license in good standing. Minimum one (1) year of nursing experience required. Home care, community health, hospice, long-term care, rehabilitation, or case management experience preferred. Strong assessment, organizational, and communication skills. Ability to work independently and manage multiple priorities. Proficiency with electronic documentation systems. Valid driver's license and reliable transportation. Current CPR certification. $45.21 - $54.45 per hour
