Lowell, MA Job Details Per diem $50 - $100 an hour 9 hours ago Benefits Flexible schedule Qualifications Home care experience Spanish RN License Working with individuals with disabilities Driver's License Driving Direct support Home health Full Job Description The agency is seeking a bilingual Spanish-speaking RN with experience in home care to work one-on-one with clients in the adult foster care program. Per diem positions and flexible hours are available. We have current job openings in the Lowell and Lawrence (Northshore) areas. Job Description Provide services to participants in accordance with the State Nurse Practice Act, agency's policy, and accepted professional standards of practice. Provide nursing services, utilize a comprehensive base of nursing theory and nursing process, and communicate/document observations and assessments. Qualifications Minimum of one (1) year of Home care experience. Graduate of an accredited school of professional nursing. Current license to practice as a Registered Nurse in MA. Minimum of two (2) years of recent experience in the direct care of elders or adults with disabilities or equivalent experience. Demonstrates written, verbal, and interpersonal communication skills. Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Has a good driving record. Essential Functions 1. Complete and coordinate all applicable clinical assessments and clinical evaluations; 2. Develop and review on an ongoing basis each member's AFC plan of care; 3. Provide oversight to AFC caregivers in conjunction with the care manager; 4. Review the PCP Summary Form; 5. Monitor the health status of all members to ensure that all needed AFC is adequately delivered; 6. Report changes in the health status of any member to the member's PCP; 7. Performs other related duties and responsibilities as deemed necessary. Good organizational and communication skills. Assessments, training, and education.
