Summary We are seeking a dedicated and highly skilled Registered Nurses (RN) and Licensed Practical Nurses (LPN) to join our compassionate team at Team Select Home Care in Massachusetts. In this role, you will play a vital part in enhancing the well-being of our patients through individualized care, making a meaningful difference in their lives.

Days:

Mondays, Wednesdays, and Fridays:

7:00 AM

• 7:00

PM Weekends :

8:00a

• 9:00p (can be split into two shorter shifts)

Overnights :

10:00p

• 6:00a Responsibilities Provide high-quality nursing care to patients in their homes, in school, and in the community.

Administer medications and all other treatments as prescribed. Provide nursing interventions for patients with feeding tubes, tracheostomies, mechanical ventilators, CVL care/TPN administration, among others. Monitor and document patient progress and vital signs. Collaborate with families and healthcare professionals to ensure comprehensive care. Educate patients and families on health management and care procedures. Maintain compliance with HIPAA regulations and company policies. Participate in ongoing training and professional development. Qualifications Valid nursing license in the state of practice. Proven experience in critical care, hospital medicine or emergency medicine. Experience in home care or inpatient pediatric nursing is preferred, but not required Strong knowledge of patient care standards and nursing practices. Excellent communication skills for effective collaboration with patients, families, and healthcare teams. Ability to perform basic life support and advanced nursing skills (e.g., IV insertion, medication administration). Familiarity with EMR/EHR systems for accurate medical documentation.
